Everything You’ll Need to Know

Everglades College Inc. is pleased to offer four different plans through UMR medical plans to fit your and your family’s needs. Starting January 1, 2025, you will have a single point of contact for all your health and welfare needs.
• Base POS $1,000-0% Plan
• POS $1,000-20% Plan
• HDHP HSA $4,000-20% Plan
• HMO $5,000-0% Plan

 

What is the overall deductible? Generally, you must pay all the costs from providers up to the deductible amount before this plan begins to pay. Suppose you have other family members on the plan. In that case, each family member must meet their deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.

Are there services covered before you meet your deductible?
Yes. Preventive care services are covered before you meet your deductible.

What is the out–of–pocket limit for this plan?
The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they must meet their out-of-pocket limits until the overall family out-of-pocket limit has been met.

What is not included in the out–of–pocket limit?
This plan doesn’t cover penalties, premiums, balance billing charges, and health care. Even though you pay these expenses, they don’t count toward the out-of-pocket limit.

Employer Contribution increases with tenure
• Tier 1 – up to 12 months
• Tier 2 – 13 months to 3 years
• Tier 3 – 3 years

NOTE: Bi-weekly payroll deductions do not cover all expenses such as copays and deductibles.
Imputed Income will be added to employee earnings for employees who provide healthcare to domestic partners and children over the age of 25.
