Contact the Call Center to complete your Enrollment process within 30 days from the date of hire or date of status change at 561.221.1493 M- F from 9 AM – 8 PM EST. After completing your enrollment, upload dependent documentation through PlanSource or email the documents to the Benefits Department at [email protected]! You must take action!

To Self-Enroll through PlanSource go to https://benefits.plansource.com > Your temporary password is your date of birth in “YYYYMMDD” format.

❖Your user credentials were sent by email from [email protected]. Note there is a dash between “no” and “reply”.                                                                     ❖Anyone hired after this date would refer to their new hire credentials email from PlanSource.                                                                                                                                ❖You can modify your benefit elections only during active Open Enrollment or if you experience a Life Event.                                                                                                          ❖ For assistance, submit a ticket to HR Help Desk at https://helpdesk.keiserschools.net/user

401K: At the 30-day mark, you are automatically enrolled in the 401K for 3%. You can change your deferrals at any time throughout the year. However, we do recommend that if you want to opt out completely, do so within 30 days of your hire date.  If you would like to make changes to your deferral, please contact Empower or reach out to the Benefits Department.

Follow these steps to download the 1095-C electronically:

• Login to your PlanSource portal: https://benefits.plansource.com>Navigate to “Documents” on the left toolbar of your portal > Select “Tax Documents”
